Waiver Program- Home and Community Based Services
Home-based care coordination that utilizes up to six different services:
- Individual care coordination
- 24-hr crisis response
- Intensive in-home services
- Family support
- Skill building
- Respite

Serves Severely Emotionally Disturbed (SED) children, ages 5 - 17 whose service and support needs cannot be met by just one agency/system and who are at imminent risk of admission to a psychiatric hospital or an institutional level of care (residential).
