Nursing Home Transition and Diversion Medicaid Waiver Program
Initiated by New York State in 2007, this program was designed to promote aging-in-place by providing service coordination, respite, independent living skills training, structured day program, positive behavioral interventions and supports, community integration counseling, home and community support, community transitional services, environmental modifications, assistive technology, congregate and home delivered meals, respiratory therapy, moving assistance, nutritional counseling/educational services, home visits by medical personnel, wellness counseling, and peer mentoring for eligible individuals.
Who is Eligible?
Persons aged 18 or older who are eligible for a nursing home level of care, as well as for Medicaid coverage, can apply for this program. Persons who wish to live in the community, rather than reside in a nursing home, can apply.

New York State recently introduced the New York Prescription Saver (NYPS), a free prescription discount card for New York State residents who are 50 to 64 years of age or persons with a disability, regardless of age. The income limits are $35,000 for a single person and $50,000 for a married person. Persons with Medicaid coverage are not eligible.
The NYPS card can lower prescription costs by up to 60% for generic drugs and 30% for brand name drugs. You only need to present the card to receive a cash discount at the pharmacy counter. As many as 3,900 pharmacies and pharmaceutical manufacturers are participating.
You can apply for the NYPS card online at http://nyprescriptionsaver.fhsc.com. You can download the application and mail it in, or call the toll-free Helpline at 1-800-788-6917. (TTY 1-800-290-9138). The names of participating pharmacies and discounted drug prices can be found on the website.
Medicaid Buy-In Program
The Medicaid Buy-In program offers Medicaid coverage to people with disabilities who are working, and earning more than the allowable limits for regular Medicaid, the opportunity to retain their health care coverage through Medicaid. This program allows working people with disabilities to earn more income without the risk of losing vital health care coverage.

Family Health Plus
Family Health Plus is a public health insurance program for adults who are aged 19 to 64 who have income or resources too high to qualify for Medicaid. Family Health Plus is available to single adults, couples without children, and parents with limited income who are residents of New York State and are United States citizens or fall under one of many immigration categories.
Family Health Plus provides comprehensive coverage, including prevention, primary care, hospitalization, prescriptions and other services. There are minimal co-payments for some Family Health Plus services. Health care is provided through participating managed care plans in your area.
